Item A Research to Determine the Ethical Climate Perception of Nurses Who Work In Private Hospitals(2021) Avçin, Emel; Can, Şeyda; Erkoç, Beyza; Yeşil, Fatma; Erdoğan, GürkanObjective: It’s imperative that organizations that want to exist in the business world where competition is intense and that want to ensure should create a positive ethical climate perception. The aim of this study was to determine the level of perceptions of the ethical climate of nurses who work in a private hospital and to determine whether ethical climate perceptions differ according to demographic characteristics. Methods: This is a descriptive cross-sectional study. The study was conducted between July-September 2018 on 154 nurses working at two private hospitals (in Istanbul and Yalova). Research data were collected from nurses working in these hospitals. In data collection, Information Form and Hospital Ethical Climate Scale were used. Results: According to findings, it was determined that the ethical climate perception of the participants was generally positive. In dimensions, ethical climate perception was found most positive in patient dimension, while most negative in physicians dimension. At findings, the perception of ethical climate according to the gender of the participants differed statistically in the managers dimension and perception of ethical climate according to working time in hospital differed in patients dimension(p<0.05). Perception of ethical climate didn’t differed significantly with age, educational and marital status of participants (p>0.05). Conclusions: It was determined that the hospital ethical climate perceptions of the nurses in the study were highly positive, and this perception was influenced by gender and the year of work. The subdimension, where the ethical climate perception was the most negative, was the physician dimension. It’s thought that the results obtained can be a guide in supporting positive ethical behavior.Item Analysis of Knowledge, Beliefs, and Attitudes of Patients in the Emergency Service Toward Rational Drug Use(2021) Can, ŞeydaBACKGROUND/AIMS: Irrational drug use results in the inability to get the expected outcome from the treatment and inappropriate demands of patients stemming from overuse of drugs and drug addiction, and these factors lead to an increased number of applications to health institutions. This study aimed to identify the factors that affect the knowledge, beliefs, and attitudes of patients presenting to the emergency department in terms of rational drug use. MATERIALS and METHODS: This is descriptive cross-sectional research that includes patients who applied for treatment to the emergency department of a state hospital in Turkey between 01.08.2019–31.12.2019. The study sample was composed of 262 patients who agreed to participate in the study. The study used the Rational Drug Use Scale (RDUS) and Drug Use Health Belief Scale (DUHBS) as data collection tools. RESULTS: The mean scores of participants from RDUS (32.37±6.56) and DUHBS (144.37±19.68) were at a good level. A positive and significant relationship was found between RDUS and DUHBS mean score of patients (p <0.01). Knowledge of rational drug use differed according to age, gender, economic status, educational level, and reasons for applying to the emergency department variables (p <0.05), and health beliefs regarding drug use varied according to the gender variable (p <0.01). CONCLUSION: Study results are significant since they revealed the factors that affect the patients’ knowledge of rational drug use and their related health beliefs, as well as the relationship between these factorsItem Examining the Stress, Coping and Trust Levels of Turkish Healthcare Professionals During the COVID‑19 Pandemic Process(2021) Can, Şeyda; Avçin, EmelThis study was conducted to evaluate Turkish healthcare professionals’ demographic characteristics and the efects of COVID-19 pandemic and stress, coping and trust levels, and to examine the correlation between them. The study has a descriptive and correlational design. The sample of the study included 529 healthcare professionals who volunteered to participate in the study between 01.06.2020 and 31.07.2020 in Turkey. Data were obtained using self-applied online questionnaires. The Perceived Stress Scale and the Trust in Relationships Scale were used to collect the data. The study found a signifcant correlation between the demographic characteristics of the healthcare professionals such as age, marital status and having children, and their perceived stress and coping level (p<0.05). The study found signifcant correlations between the variables of having someone around them diagnosed with COVID-19, contacting with a COVID-19-suspected/positive patient, having a chronic disease, needing psychological support, questioning interpersonal relationships, quality of life and looking to the future with hope/enthusiasm, and healthcare professionals’ perceived stress and coping levels (p<0.05). There was a signifcant correlation between the variables of having a chronic health issue, abiding by the rule of stay at home/social isolation, quality of life and looking to the future with hope/enthusiasm and the Trust in Relationships Scale (p<0.05). Determining the perceived stress, coping and trust levels of Turkish healthcare professionals should be regarded as an opportunity to provide necessary support not only in this pandemic period but after all this end.Item ONKOLOJİ BİRİMİNDE TEDAVİ ALAN HASTALARIN MANEVİ İYİ OLUŞLARI VE YAŞAM KALİTELERİ(2021) Köktürk Dalcalı, Berna; Durgun, Hanife; Can, ŞeydaManevi iyi oluş kişinin hayatındaki dengeye işaret etmektedir. İnsanları yaşamları boyunca karşılaşabilecekleri bazı hastalıklar bu dengeyi bozabilmektedir. Özellikle kanser gibi yaşamı tehdit eden hastalıklarda bireyin yaşam kalitesinin sürdürülmesinde manevi iyi oluşun rolü büyüktür. Bu nedenle çalışma kanser hastalarının manevi iyi oluşları ve yaşam kalitesi arasındaki ilişkiyi belirlemek için gerçekleştirilen kesitsel bir çalışmadır. Araştırma Kasım 2019-Şubat 2020 tarihleri arasında çalışmaya katılmayı kabul eden 87 hasta ile tamamlanmıştır. Veriler araştırmacılar tarafından geliştirilen yapılandırılmış soru formu, Manevi İyi Oluş Ölçeği (FACIT-Sp-12) (Version 4) ve Yaşam Kalitesi Ölçeği (EORTC QLQ-C30) ile toplanmıştır. Veriler SPSS 21.0 programı ile analiz edilmiştir. İstatiksel testlerden yüzde, frekans, ortalama, standart sapma, iki bağımsız değişken arasındaki farklılığın belirlenmesinde Student t testi ile çoklu bağımsız değişkenler arasındaki farklılığın belirlenmesinde ANOVA testi kullanıldı. Veriler arasındaki ilişkinin değerlendirilmesinde Pearson Korelasyon Analizi kullanıldı. Hastaların manevi iyi oluş puanlarının ortalamanın üstünde olduğu görüldü. Çalışmada hastaların; Manevi İyi Oluş Ölçeği’nden aldıkları puanların yüksek olduğu, EORTC QLQ-C30 alt boyutlarından aldıkları puan ortalamaları; Avrupa ülkeleri referans değerleri ile karşılaştırıldığında semptom skorunun Avrupa ülkelerinin referans değerinin üzerinde, fonksiyonel skor ve genel sağlık skorunun ise referans değerin altında olduğu görüldü. Manevi iyi oluş toplam puanının fonksiyonel skor ve genel sağlık durumu skoru ile ilişkili olduğu görüldü.Item Pain Assessment in Patients Who Receive Hemodialysis Treatment(2021) Can, Şeyda; Arda, ArzuAim: Pain is an important health issue that is common among patients who receive hemodialysis treatment and greatly affects the patients’ quality of life. This study aims to assess the qualitative pain characteristics of patients who receive hemodialysis treatment using the McGill Pain Questionnaire. Material and Method: This descriptive and cross-sectional study included 87 patients who received HD treatment. Data were collected using an information form and the McGill Pain Questionnaire in the HD clinic of a training and research hospital in Turkey between 01.04.2019 and 31.09.2019. Results: The study is found that the mean current pain scores of the HD patients were moderate (2.13±0.56). The study is determined that the patients experienced pain most often in the lower extremity (36.8%) and head region (29.9%) and least in the upper extremity (11.5%). The hemodialysis procedure (44.8%), not following the diet (23%), fatigue (16.1%) and stress (16.1%) were found to intensify the pain. The study is found that analgesics (36.8%), resting (31%), complementary approaches (17.2%) and other practices (14.9%) relieved pain when patients were in pain. The study is also found that the patients often used the words tiring (n=47), sickening (n=42), fearful (n=41), and wretched (n=38) to define the pain they felt. Conclusion: Measuring pain are greatly important to increase the quality of life. The results obtained indicate that assessment of the pain individualistically will be a guide to provide a holistic approach in HD patients.Item Parents' Family-Centered Care Perception and Investigating Factors Affecting Such Perception(2021) Avçin, Emel; Can, Şeyda; Yeşil, FatmaObjectives: This study aimed to examine the perception of family-centered care (FCC) and the factors affecting this perception from the perspective of the parents. Methods: The study design used is descriptive and cross-sectional. The study was carried out in two public hospitals in Bursa and Yalova city centres in Turkey. The universe of the study consists of 240 families whose 0–18-year-old child is hospitalized and stays with the child for at least 3 days between the specified dates. Results: A total of 240 parents participated in the study and 177 (73.8%) were women. The median value of significance was 26.0 (20.0-34.0) in hospitalized girls and was 28.0 (22.0-33.0) in hospitalized boys (p=0.019). The median value of significance dimension for children with chronic disease was 28.0 (22.0-31.0) and 28.0 (20.0-34.0) for those without chronic disease; in the consistency dimension, the median value of children with chronic disease was 22.0 (17.0-27.0) and 24.0 (14.0-31.0) of children without chronic disease (p=0.049 and p<0.001, respectively). Matching percentage of all items in the scale changes between 68.3-80.5%. Conclusion: It was determined that nurses make parents feel that the care given to their children is good, but they need to be more supportive as provided to make the parents feel an important place in the care of the child. When FCC is applied in hospitals, the satisfaction level of the child, family, and nurse will increase.Item Primipar ve Multipar Annelerin Yenidoğan Bakımına İlişkin Geleneksel İnanç ve Uygulamaları(2021) Avçin, Emel; Can, ŞeydaAmaç: Bu araştırma, primipar ve multipar annelerin yenidoğan bakımına ilişkin geleneksel inanç ve uygulamalarını belirlemek amacı ile yapılmıştır. Annelerin sağlıkla ilgili inanç ve deneyimleri, yenidoğan bakım uygulamaları ile ilgili kararlarını ve davranışlarını etkilemektedir. Annelerin benimsediği geleneksel inanç ve uygulamalarında herhangi bir sakıncası olmayanların belirlenmesi, bilime dayandırılarak sahip çıkılması önemli iken, bu uygulamalardan zarar verenlerin (tuzlama, kundaklama, höllük, emzirmeye geç başlama, pudra kullanma vb.) tespit edilmesi ve ortadan kaldırılması da gerekmektedir. Gereç ve Yöntemler: Tanımlayıcı ilişki arayıcı nitelikte olan araştırmanın evrenini, Yalova ilinde yer alan bir devlet hastanesinin kadın doğum, doğumhane ve çocuk servisine 01.07.2020-30.12.2020 tarihleri arasında başvuran anneler (n=502) oluşturmuştur. Araştırmada örneklem seçimine gidilmemiş, araştırmaya alma ölçütlerini karşılayan tüm annelere ulaşılması hedeflenmiş ve 311 (evrenin %61,9’u) anne çalışma kapsamına alınmıştır. Araştırma verileri, araştırmacılar tarafından oluşturulan bilgi formu (15 soru) ve geleneksel inanç ve uygulamalar formu (40 soru) ile toplanmıştır. Bulgular: Annelerin primipar veya multipar olmalarına göre bebeklerinin ilk emzire zamanı, kundaklama, sarılık, kırkının çıkması, albasması, nazar, ilk banyo, pamukçuk, pişik ve göbek düştükten sonra yaptıkları geleneksel uygulamalarında anlamlı farklılık olduğu görüldü (p<0,05). Annelerin, primipar veya multipar olmasına göre geleneksel uygulamalara katılma oranlarının değiştiği belirlendi. Sonuç: Elde edilen sonuçlar, annenin doğum sayısının geleneksel uygulamalara yaklaşımını değiştirip değiştirmediğine ışık tutması nedeniyle önemlidir. Annelere ve ailelere riskli geleneksel uygulamaları ile ilgili davranışlarını önleme ve sosyal desteği artırmaya yönelik eğitim ve danışmanlık hizmetlerinin verilmesi önerilmektedir.Item Reliability and Validity of Turkish Version of the Self-administered Foot Health Assessment Instrument(2021) Durgun, Hanife; Köktürk Dalcalı, Berna; Can, ŞeydaObjective: This study aimed to examine the Turkish validity and reliability of the Self-Foot Health Assessment Tool. Material and Methods: This study was planned as a methodological type study. This study was conducted with 220 nurses working in different units in Ordu State Hospital and Ministry of Health Ordu University Training and Research Hospital. In addition to the Self-administered Foot Health Assessment Instrument, a personal information form was used to collect the demographic information from voluntarily participating 220 nurses. In the validity study of the scale, the content validity index was used to evaluate language validity, content validity, construct validity and expert opinion. In the reliability study of the scale, the Cronbach’s alpha coefficient calculation, item analysis and confirmatory factor analysis were used to determine the internal consistency. Results: The average age was 36.75±8.61 years, average working experience years as a nurse was 14.32±9.56, and the average working duration in the same service was 4.97±5.66 years. After the language equivalence of the scale was achieved, the content validity index was calculated. The Kuder-Richardson 21 coefficient was calculated as 0.784 for skin health, 0.774 for nail health, and 0.626 for foot anatomy. The content validity index score was 0.959 (excellent level). The overall internal consistency coefficient of the scale was between 0.851-0.929. Average varience extracted value was 0.404 and composite reliability value was calculated as 0.942. Conclusion: Participation of healthy/patients individuals that nurses provide care is very important. It has been determined that the Turkish version of the Self-Foot Health Assessment Tool, which can be effective in providing and evaluating the self-care of individuals, is a valid and reliable tool and can be used safely by individuals in self-evaluation of foot health.Item The relationship between the individualized care perceptions of nurses and their professional commitment: Results from a descriptive correlational study in Turkey(2021) Can, ŞeydaAim/objective: This study aimed to measure individualized care perceptions and professional commitment among nurses and investigate the relationship between these variables. Background: It is necessary to identify the factors that affect professional commitment and individualized care, which further influence outputs such as quality of care and patient and staff satisfaction. Design: It is a descriptive cross-sectional study. Methods: This study was conducted with 231 nurses working in surgery, internal diseases and intensive care departments at a public hospital in Turkey between September 2018 and March 2019. Results: The study found that the perception of individualized care among the nurses (ICS-A) was on a good level (3.81 ± 0.71) and their professional commitment (NPCS) was at on a medium level (79.95 ± 13.07). There was a positive significant relationship between the departments at which the nurses worked and their perceptions of individualized care (p < 0.05). A positive significant relationship (p < 0.05) was also found between the professional commitment of the nurses and the duration of their service at the departments and whether they chose the profession willingly (p < 0.05). The results of the study indicated a positive significant relationship (p < 0.05) between the NPCS and ICS-A mean scores of the nurses. Conclusions: This study is expected to be guiding in education and practice environments on both national and international levels in terms of increasing patient care quality, patient satisfaction and job satisfaction in nurses.
